Job Description
As a direct assistant to Social Services and Admissions in our Skilled Nursing setting, this team member will provide counseling and support to residents in all levels of care; coordinate admissions, transfers, and discharges to assure regulatory compliance, accurate documentation and an excellent resident and family experience. Keep staff informed of resident's status and condition to utilize appropriate skills, intervention assistance and support from trained and skilled staff at Seabury

Position Requirements: Education : Minimum of Bachelor's Degree in Social Work or related field.
Experience : 2+ years long-term care experience, required.
Skills and Competencies: - Ability to read, write, speak, and understand the English language clearly.
- Must be able to relate to the elderly in a dignified manner and build a relationship with residents that are positive and supportive.
- Must be extremely knowledgeable and competent in all processes of SNF admissions and discharge procedures - documentation and compliance requirements.
- Computer/technical proficiency to include Microsoft suite and ease with electronic records, etc.
- Strong organizational skills
